Lifetime Dog Licenses

All dogs three months of age or older must be licensed.

The Application Process
The dog must be permanently identified by microchip. The dog owner must complete the Lifetime Dog License Application and provide both the application and the completed Permanent Identification Form to the Lancaster County treasurer.

The Permanent Identification Form must be completed by a veterinarian and returned to the Treasurer's Office within 30 days of completion. Please make sure the form is completed in its entirety.

Instructions
When completing both the Lifetime Dog License Application and the Permanent Identification Form, remember to do the following:
  • Print application to your printer at 100%.
  • Do not scale to fit
  • Fill out the form
  • Cut on the dotted line and submit to the Lancaster County Treasurer's Office or a license agent in Lancaster County

Enclose a check or money order (No Cash Please) for the proper amount made payable to the Lancaster County Treasurer and

Upon receipt of both forms, a lifetime dog license will be issued to you. The dog must wear the license tag to be in compliance with the Pennsylvania Bureau of Dog Law.

Lost Tags
If your dog loses the tag, mail a request to the county treasurer at the address listed above, along with a check or money order in the amount of $1.45 payable to the Lancaster County Treasurer. The Pennsylvania Bureau of Dog Law will prepare a new tag and mail it directly to you within a few weeks.
